you could try to find the call for db:migrate in the puppet module under /usr/share/foreman-installer/modules/foreman and add --trace there

that way I think the output should be in the installer log

anyway, check /etc/foreman/foreman-installer-answers.yaml if you spot some old value

normally one wants the FQDn to resolve to the IP of the NIC
