in a form, I need to have a formset to select some related objects in M2M through a intermediate models

the easy way is to have a simple select box for the related model and some inputs for the additional data

but I need to have a checkbox for each possibly lined model with a set of input for additional fields

How can I achieve this ?

I see how to do it with a formset in the first case (select the related model instal in each formset)

but I can't find out how to do this with the checkboxes..
